Sign-ups for modified spring sports will run through Friday, March 6.
Available spring sports include baseball, boys' and girls' lacrosse, softball and co-ed track. Practices for baseball, girls' lacrosse and softball will be held at Stephen and Harriet Myers Middle School. Boys' lacrosse practice will be at William S. Hackett Middle School and co-ed track practice will be at Albany High School.
Students should see a physical education teacher to sign up. Tryouts begin March 16.
Required paperwork
District officials must have the following documents before a student can participate in sports: Download the documents below or pick up a packet in the nurse's office. Forms must be signed by a parent or guardian and returned to the school nurse by Friday, March 6.
- A current health appraisal form completed by the student's health care provider. This is the Required NYS School Health Examination Form. This physical must be on file in the nurse's office. A current physical is one that has been completed within 12 months of the month that the sport starts. The district encourages that an individual student receive a physical from his or her health care provider, and that the provider complete this form. If a student needs a physical and does not have a health provider, the district will schedule a physical for you.
- A health history and permission form to be completed by the student's parent or guardian. This is Seasonal Sports Interval History HE-106 form, and must be filled out before the start of each season. This form cannot be dated earlier than one month prior to the start of the season.
- If the student requires medication (inhaler, epi-pen etc.) during sports, the HE 132 Medical Permission form and the HE 133 Self-Administered Medication Permission form must be completed and returned in order to be cleared. This form has to be current for each school year and on file in the nurse’s office.
In addition, the Seasonal Sports Interval History form contains a concussion information sheet that parents/guardians must review and keep for their records.
Students will not be allowed to play a sport if their required paperwork is not turned in by March 6.
