The City School District of Albany is providing a consent form for parents and guardians in preparation for the possibility that the district may be required to test in-person students and employees if the current COVID-19 surge continues.
Parents and guardians of in-person students can complete the consent form online. A paper version also will be sent home with students.
Parents and guardians opting for the online version can log in with their student’s district Google credentials. Those opting to complete the paper version should return the completed form to their child’s school.
If all or part of the Capital Region is designated an orange or yellow zone in alignment with the state’s micro-cluster strategy for containing COVID-19, the district would conduct random testing each day on a percentage of in-person individuals at all buildings, including district offices at Academy Park, Essex Street and Harriet Gibbons Student Services Center.

The district is monitoring daily COVID-19 data and developments closely and continues to move forward with regular schedules for in-person and virtual instructional models. The district is not required to conduct COVID-19 testing at this time.
